Monday, April 29th - Team / Individual Photos! * IMPORTANT DETAILS *

@ Lower Centennial Hall - 311 South Shore Road

BEFORE APRIL 29th:

• Watch your RAMP App (email for BLAST BALL) for your team's ARRIVAL TIME.

• RSVP to your Team Manager via RAMP App (email for BLAST BALL)

• Team Managers will distribute order form envelopes as soon as they become available. 

• LCDMBA will provide ONE complimentary Memory Mate (shown on front of envelope) to each player at no charge.

• If more than one PARENT wishes to purchase photo products, please request an additional envelope from your Team Manager.

• Prior to arrival - even if you do not intend to purchase any products, complete the form on back of envelope.

  • Required: PARENT Name, phone, email

  • Required: PLAYER's First Name, Last Name, Division (eg: 9U / 11U), Team (eg: Lakers, Cardinals, Jays)

  • OPTIONAL PLAYER INFO: Age, Height, Weight, Position, Coach (These will only appear on some products if chosen for purchase)

• PAYMENT: EXACT CASH (no change will be given), CHEQUE payable to Freeze Frame, or CREDIT CARD info written on the form.

ON APRIL 29th:

• All players are expected to arrive AT THEIR DESIGNATED ARRIVAL TIME.

• Parents must supervise their children at all times when outside of the building.

• Photo participants will enter through the Lower Centennial Hall doors and follow instructions from volunteers.

• ONLY Players and Team Staff (one additional adult per player - if required) will be permitted inside the Lower Hall.

• Players will be expected to SIT in the designated area for their team - absolutely no running, yelling or screaming!

• Photos will take place in the Upper Hall, but ONLY the team being photographed and the next team will be permitted upstairs.

• Teams will return back down to the Lower Hall together after their photos are complete.

• Parents must collect their children and exit the Lower Hall.
